I certify that to the best of my knowledge and belief, the answers given by me to the foregoing questions and the
statements made by me in this application are correct and complete. I understand that any false information,
misrepresentation, or material omission of information contained in this application or any document to secure
employment may result in rejection of this application or immediate discharge if I am employed.

 

I hereby authorize Omega Hotel Group to communicate with my former employers, school officials, and persons named as
references and investigate any other matters related to my suitability for employment and, further, authorize my
former employers to disclose to Champion Hotels any and all letters, reports and other information related to my
work records, without giving me prior notice of such disclosure. In addition, I hereby release Omega Hotel Group, my
former employers, and all persons, corporations, partnerships and associations from any and all claims, demands or
liabilities arising out of or in any way related to such investigation or disclosure.

 

I understand that, if employed by Omega Hotel Group, I may be requested to work overtime hours or hours outside a
normally defined work day or work week. I may be required to supply proof of a valid driver's license as it pertains
to my employment. I may also be required to drive my own vehicle(s) to and from job sites, and agree to carry
liability insurance on the said vehicle(s) as required by law.

 

It is not the policy of Omega Hotel Group to enter into employment contracts unless they are in writing and signed by
the Human Resources Manager, President, COO, CFO or OUM. Accordingly, I understand that, if hired, my employment and
compensation can be terminated at-will with or without notice, with or without cause at any time, at the option of
either myself or Omega Hotel Group. I understand and acknowledge that no employee or representative of Omega Hotel
Group, other than the Human Resources Manager, President, COO, CFO or OUM specifically approving a written contract
in writing, has any authority to enter into any agreement for employment for any specified period of time, or to
make any agreement contrary to the foregoing. Employment by Omega Hotel Group is conditioned upon the
applicant’s execution of Omega Hotel Group’s offer letter, the acknowledgement and receipt of Champion
Hotel’s Personnel Policy Manual, passing of drug test if required, and having an insurable driving record and
valid driver’s license where required.

 

I also understand that all offers of employment are conditioned on the provision of satisfactory proof of my identity
and legal authority to work in the US.

 

Lastly, I understand that my employment with Omega Hotel Group and its affiliates is contingent upon passing the
background investigation. If the outcome is unfavorable, employment can be rescinded and/or terminated.
